The following required forms MUST be completed, and uploaded, before you are able to coach in our Rec or Travel programs.   ALL FORMS MUST BE CURRENT AND UPLOADED BEFORE THE FIRST PRACTICE. 

Required clearances/forms are: 

• Act 34 Criminal Background Check (PA State Police Background Check) - Every 5 years
• Act 151 Child Abuse Clearance (PA Child Abuse) - Every 5 years 
• Act 114 Fingerprint Clearance (FBI Criminal Background Check) - If you have lived in PA less than 10 years or an affidavit if you have lived in PA for more  than 10 years. 
• Concussion Awareness Training (Head's Up) - Annually 

Travel coaches must also complete Safe Sport Training and Risk Management Annually. 

All trainings are located in our GotSport online platform.  Coaches will be emailed login information and directions. 

Please remember that this is mandatory for all coaches.  We have adopted a zero tolerance policy and you will not be allowed to coach in practice or games if your current certifications/forms are not uploaded and active.
